Requirements
- Proof of identity and right to work; W-9 (or W-8BEN if applicable).
- Background check authorization (helpers/sitters and concierge associates).
- Active insurance and COI naming Maywood Holdings LLC (contractors).
- CT Home Improvement Contractor (HIC) registration for covered trades; trade licenses as applicable.
- Acceptance of independent contractor ICA or Concierge ICA (as applicable).
